Where can I park?
Montague Quarter Car Park is a 3 minute walk away. Multiple spaces. It costs £1.10 for 1 hour, £2.20 for 2 hours etc.
On street parking on nearby Liverpool Gardens and Shelley Road. Pay and display machines at £1.30 for 1 hour, £2.50 for 2 hours.
What do I need to bring with me?
The medical form that relates to you medical assessment / certificate.
Photographic ID with your correctly corresponding address.
If you wear glasses or contact lenses for driving then please bring these with you. If you have a copy of your most recent prescription (glasses/contacts) then bring this with you.
What happens if I cancel?
If you cancel more than 24 hours before your scheduled appointment then the cost is refunded to the card you used as payment (minus the booking fee). If you cancel less than 24 hours before your scheduled appointment then no refund will be issued.
In the rare situation that we may need to change your appointment you will be offered a rescheduled appointment or refunded in full.
Unfortunately, we do not currently offer the following medicals:
Taxi medicals
Oil & Gas medicals
Cruise ship pre-employment medicals
Safety Critical medicals
ENG1 Seafarer medicals

What happens if the doctor is unable to complete the medical?
This occasionally happens when the medical reveals an unforeseen health issue that requires further tests and/or treatment. If you ‘fail’ a medical due to high blood pressure - you will be rescheduled at a later date without incurring any further costs. For all other issues - see our T&Cs page.
What happens once the medical is completed?
Post your completed D4 medical form along with your application form and your photo ID driving licence to DVLA, Swansea, SA99 1BR.
For a Motorsport medical - post your completed form, payment, photo ID +/- ECG to Motorsport UK, Bicester Motion, OX27 8FY.
Fishing medical - Take both the ML5 report and certificate to your local MCA Marine Office or RYA for the commercial endorsement.
